Health Savings Account
Enrollment in High Deductible Medical Plans 6 or 7 allows employees to participate, if they choose, in a Health Savings Account (HSA).
EPSD9 will contribute $75 per month in addition to the amount the employee chooses to contribute monthly ($0- max $4,150/year for an individual or $8,300/year for a family (2024)).
HSA providers for EPSD9 are American Fidelity and OPTUM.
Upon enrollment in Medical Plans 6 or 7, you will receive a form via Frontline to begin enrollment into the HSA of your choice. After this Frontline form is completed, an additional form directly from the provider may be required to complete the enrollment process.
Once enrolled in an HSA you can make changes to your HSA by using the same Frontline form. Access the form while logged into Frontline, select "My Forms / Forms I can start" and choose the "HSA payroll deduction form", then submit it after it is completely filled out.
